Children in Need

Children in Need work with children with additional needs, such as complex health needs and disabilities. Each team within the service has a particular function, some of which are related to a particular age.

Portage is a home visiting service for children of pre-school age. We work closely with families and their children at the point of diagnosis either at birth or when the children are toddlers. Our aim is to help parents to understand the diagnosis, and support and teach them how to deal with it.

Multi Agency Planning Pathway (MAPP) works with children who have the most complex health or disability needs. We co-ordinate care in the form of multiple services to the children.

Inter Agency Transition Team (IATT) works with young people between the ages of 14 and 25. Our job is to co-ordinate the effective transition form children's services to adult services.

Children with Disabilities works with children and their families through the age range 0-18, undertaking assessments of need and providing ongoing social work support and/or services.

Community Occupation Therapists provide assessments to determine which aids and adaptations are needed within the child's home environment.

Meliot Road Family Centre works with families where there is a need to assess the parenting capacity. We work with parents and their children to implement a parenting programme.

Social Workers in CAMHS provide a service to children and young people where a mental health need is identified.
